APPLICATION INSTRUCTIONS

To apply to be a delegate to 2012 Texas Boys State, please read these directions first:

  • Step 1: If you do not have Adobe Reader or Adobe Acrobat for viewing PDFs, please download Adobe Reader here for free before you begin.
  • Step 2: Click on the “Texas Boys State Application” link below.
  • Step 3: You will be taken to the Acrobat.com site where the form is stored. This is a secure, encrypted site.
  • Step 4: You may either DOWNLOAD the form or OPEN the form in your browser by clicking on one of the two green buttons in the middle of the screen. Either button will work.
  • Step 5: The PDF form will open. Fields will be highlighted (probably in light blue) for you to type directly into. Fill out the one-page form by typing directly into the fields. At minimum, you must fill in the required fields in the APPLICANT and PARENT OR GUARDIAN sections (these required fields will probably appear outlined in red). You may fill out the fields in the other parts of the form (Physician, School Official, Legion Information) if you know the information–this keeps the form looking neat and typed when you print it. If you don’t know the information for the other parts of the form, leave them blank.
  • Step 6: Once you have typed in the information, click the SUBMIT FORM button in the upper right hand corner of the form (in recent versions of Adobe Reader, this button appears on a purple toolbar). You will be prompted to enter your name and email address. Then click SEND. You will see a note on your screen that it was submitted successfully.
  • Step 7: Next, PRINT the filled-out form or save a copy of the form to your computer to print later.
  • Step 8: Get signatures from everyone that is needed to complete and sign the form: APPLICANT, PARENT/GUARDIAN, PHYSICIAN, SCHOOL OFFICIAL, and LEGION OFFICIAL.
  • Step 9: When the form is complete and all signatures are obtained, make a copy of the form for your records. Then, the APPLICANT or PARENT/GUARDIAN should mail the ORIGINAL form to:

Common questions and technical problems regarding the application process:

  • I can’t open the application. The application is a PDF file. To open it, you need a recent version of Adobe Reader, a free program you can download here.
  • I don’t see a Submit Form button on the form. This button appears in the upper right hand corner of the form, on a toolbar that is usually purple. If you don’t see this, open the PDF form. Go to Edit > Preferences > Forms. Make sure the “Always hide forms document message bar” check box is NOT checked. Click OK. You may need to close the form and re-open it. If that still doesn’t work, make sure you have a recent version of Adobe Reader installed on your computer. You may also want to try a different browser or different computer.
  • What if I don’t know some of the information I need to enter on the form? At minimum you should fill in the APPLICANT and PARENT/GUARDIAN sections of the form by typing into the fields. Once you have filled out these two sections, you may SUBMIT the form online (see Step 6 above). Then, when you print the form, you’ll have these two sections neatly typed into the form. You can take the form to your physician, school official, and Legion official, and they can each hand-write their information into their corresponding section. If you know the contact information for your physician, school official, and Legion official, though, you may enter it into the form before you submit it online. This way, you’ll have all of their information typed neatly into the form and all they will need to do is complete their sections and sign it.
  • My school counselor or Legion official gave me a paper form to fill out and it looks completely different from the form you have posted online. Which one should I fill out? We have changed the application form and process for 2012. You probably have an old form. You MUST fill out the APPLICANT and PARENT/GUARDIAN sections of the NEW FORM online and SUBMIT the form ONLINE. After this, you then print the form and take it to your physician, school official, and Legion official for them to complete and sign the form. And only after all of that is done should you mail in the form.
